Case Management Services for Children and Youth Ages: 17 and under

Case Management is a voluntary in-home intervention and advocacy service. It is designed, whenever possible, to keep children and families together. The goal is to teach the child and families the skills they need to be stronger and healthier, and to be able to advocate for the child's needs. Case Managers link, refer, advocate and skill-build for families in the communities where they live.
Eligibility: Must be under 18 with a mental health diagnosis or a severe emotional disturbance. Has a history of out of home placements or difficulty in the home environment. Has difficulty adjusting to their natural environment - school, home, and/or community.
