BeWell Primary Care Wrap Around

BeWell provides patient education, primary care services, health care coordination, health coaching and peer support for individuals with both a serious mental illness and a physical health challenge. Participants focus on health goals such as smoking cessation, maintaining an active lifestyle, weight loss or reduction in cholesterol or blood pressure.
