Under one roof, medical providers work hand-in-hand with Centerstone to identify children who may be experiencing behavioral health symptoms, complete a comprehensive assessment and develop a treatment plan. HIV and Substance Use Prevention provides age-appropriate substance use and HIV prevention services to at-risk young adults. This free program is provided through a grant from the Substance Abuse Mental Health Services Administration/Center for Substance Abuse Prevention.
Centerstone provides comprehensive psychiatric evaluation and medication management to adult, adolescent and child clients with substance abuse and psychiatric diagnosis. Evaluations and ongoing medication management are done by staff psychiatrists and clinical nurse specialists.
Health Navigator services include care coordination and coaching around physical health issues for adults, youth and their families. The focus of the work may include preventative care, screening and management of health issues such as asthma or diabetes.
BeWell provides patient education, primary care services, health care coordination, health coaching and peer support for individuals with both a serious mental illness and a physical health challenge. Participants focus on health goals such as smoking cessation, maintaining an active lifestyle, weight loss or reduction in cholesterol or blood pressure.
