We work closely with you to connect you to resources to help you manage your chronic physical health concerns as well as your mental health conditions.
We know that mental health care is not a one-size fits all. There are varying levels and even types of need. Some in our communities require a dedicated set of services. With a range of services including psychiatrists in addition to our other providers, primary care coordination and even special programs designed for those who are involved in the criminal justice program, more people can get the care they need to live their best lives. We design services to serve unique populations with unique needs in our communities.
Managing your mental health can be difficult when you have ongoing physical health concerns or chronic health conditions. We work closely with you to connect you to resources to help you manage your physical health as well as your mental health. By focusing on “whole person” care, you are more likely to see better outcomes and feel better faster.
Depending on the level of care you need, a psychiatrist may work with your case. Treatment can include inpatient hospitalization, outpatient therapy, or medication management. Our team will work with you to determine which combination is best for you. Some of the conditions we treat include depression, anxiety, schizophrenia, PTSD and ADHD.
We provide safe, supportive services for those in the criminal justice system. With the help of local courts and other community partners, we want to ensure that people who are leaving correctional institutions experience a smooth transition back into their communities.
