Certified Community Behavioral Clinics (CCBHC’s) represent a patient-centered approach to mental heal, substance use, and primary care, offering comprehensive services to all patients regardless of insurance status or ability to pay. This model furthers Centerstone’s mission of delivering care that changes people’s lives.

What CCBHCs Offer

Funded through SAMHSA grants of Medicaid demonstration programs, Centerstone’s CCBHCs deliver nine essential series either directly or through formal partnerships.

Whole Person Care

CCBHCs integrate mental and physical health care, ensuring comprehensive healing by addressing all factors impacting well-being. They improve access to care for everyone regardless of location, age, or ability to pay.

Nationally, CCBHCs have reported great success. This includes reductions in hospitalizations and homelessness and less time in jail. And as a CCBHC, Centerstone is part of this positive change. As a CCBHC, Centerstone is not just doing more — we’re doing better.

Centerstone’s Bradenton Clinic in Bradenton, Florida received its first two-year CCBHC-E grant in 2020 funded by The Substance Abuse and Mental Health Services Administration (SAMHSA). Currently, using the CCBHC Improvement and Advancement Grant awarded in 2022, the clinic continues to transform community behavioral health systems and provide comprehensive, integrated, coordinated, and person-centered behavioral health care. Additionally, in October 2023, Centerstone received a Planning, Development, and Implementation Grant to create a CCBHC in Ft. Myers, Florida which will help fill key service gaps in the area and target crisis needs.

Services are made available to any individual, children, adolescents, and adults, seeking care for a mental and/or substance use disorder, in addition to individuals experiencing a mental health or substance use-related crisis. Through CCBHC funding, the Bradenton Clinic has increased capacity by hiring more providers. This includes nursing staff at the walk-in center to aid in addressing any physical health care needs. Identified as an area of need in our 2023 community needs assessment, the clinic staff work to better serve groups facing unique health disparities such as individuals with co-occurring disorders, individuals from minority groups, and individuals with limited English proficiency.

Our customer care staff are familiar with our payment options and will ask questions to determine if you qualify for grant funding or need-based payment assistance programs. Our staff can explain options over the phone. Assistance is made possible through charitable donations to Centerstone’s Foundation, local and federal grant programs, and a sliding fee scale which is available to all clients but only at certain sites with National Health Service Corps certification. Eligibility may vary by state.

The Centerstone clinic in Alton, Illinois received its first CCBHC expansion grant in 2018. The second CCBHC Improvement and Advancement (IA) Grant was awarded in 2020 and ended in May of 2023. In October 2023, the Alton Clinic received another CCBHC IA grant which will support services until 2027. Additionally in October 2023, the Carbondale Clinic received their first Planning, Development, and Implementation (PDI) grant which will allow this clinic location to expand services and better serve the local community’s behavioral health care needs.

Our implementation and expansion is focused on ICM – Intensive Case Management, which employs peer specialist, counselors, and clinicians that work to support clients in accomplishing long-term case goals and outcomes over relatively short periods of time.

CCBHC funding also supported the creation of telehealth rooms in our facility prior to the COVID-19 pandemic which was instrumental to our readiness for virtual care. We started experimenting with using telehealth therapists at the start of the pandemic and have found much success utilizing this mode of care delivery when necessary.

Case management, individual and group wellness coaching, and peer support services not covered by insurance have also been supported by the CCBHC grant. Our MAIN goal has been working with the State Mental Health Association to become a demonstration state which will ensure continued success of the communities and clients in which we serve.

While working on developing a state-funded CCBHC model, Centerstone will continue expanding and making improvements to services through the CCBHC IA Grant at the Alton Clinic and CCBHC PDI grant at the Carbondale Clinic through 2027.

The Centerstone Clinic in Bloomington, Indiana received its first CCBHC expansion grant in 2018, its second in 2020, and now utilizes the CCBHC-IA (Improvement and Advancement) Grant, which was awarded in 2022 and provides funding through 2026.

Through the three CCBHC SAMHSA grants over the past 5 years, the Bloomington clinic has facilitated a shift toward integrative health care (in-line with CCBHC requirements), which we, as an organization, call whole health care. CCBHC grant-funded staff have enabled the delivery of whole health care services at Centerstone through the provision of health coaching, life skills, health education (individual and groups), and general health assessments & reassessments for all clients. CCBHC grant-funded staff support our walk-in clinic (Same Day Access) through the provision of intake assessments, therapy, case management, medical record access/coordination, and integrated care coordination (both internally and externally), especially for those who are un-/under-insured. Additionally, CCBHC grant funds are used to provide hospital inpatient intake assessments, as well as intake assessments for probation & problem-solving court clients. At the administrative level, CCBHC grant staff are engaged in numerous whole health workgroups and projects, which are supporting and guiding Centerstone of Indiana in the implementation and delivery of whole health care.

The Harriett Cohn Clinic in Clarksville, Tennessee received its first CCBHC expansion grant in 2020. The second CCBHC Improvement and Advancement Grant was rewarded in 2022 and is ongoing through 2026. In 2023, the Tullahoma Clinic located in Tullahoma, Tennessee received their first Planning, Development, and Implementation grant which will be ongoing until 2027.

The CCBHC project has supported the implementation and expansion of Medication Assisted Treatment (MAT) at the facility by employing full-time providers and nurses. MAT services are offered in-person and via telehealth. Case management, individual and group wellness coaching, and peer support services not covered by insurance have also been supported by the CCBHC grant. Current funds are being used to expand and sustain substance use treatment and follow-up care post-rehab, including Intensive Outpatient (IOP) group therapy, particularly for underinsured populations.
