Older Adult Services
Centerstone’s Older Adult Program is offered in Davidson, Robertson, Rutherford, Williamson, and Wilson County, Tennessee. This program provides supportive care management to individuals age 50 and older who have mental health needs. Assistance ranges from health care coordination, service referral, and advocacy related to financial, housing, legal, and safety matters. Additionally, these services provide support to our client’s family members and/or caregivers and is available at no charge.

Eligibility
This program is available at no cost to individuals 50 years of age or older, who are not enrolled in TennCare/Medicaid or the Behavioral Health Safety program. Participants may have Medicare, Commercial, or Private Pay insurance. Associated family members and caregivers are eligible to receive services as part of an individual’s care coordination plan.
The project is funded under a Grant Contract with the State of Tennessee Department of Mental Health and Substance Abuse Services
