Older Adult Services

Centerstone’s Older Adult Services in Davidson County, TN, provides supportive care management to individuals age 50 and older who have mental health needs. Assistance ranges from health care coordination to linkage, referral and advocacy related to financial, housing, legal and safety matters. This grant-funded service also provides support to the client’s family members and/or caregivers and is available at no charge.

Eligibility

This service is available at no cost to qualifying individuals who are age 50 or older, reside in Davidson County, TN, and do not have TennCare or Safety Net health insurance plans. Participants may have Medicare or private pay coverage. Associated family members and caregivers are also eligible to receive services.

Centerstone’s Older Adult Services are funded by a grant from the Tennessee Department of Mental Health & Substance Abuse Services.
